Care, a service of Health Network partner, is a program developed by Hospital and its Board of Trustees, as a way to provide medical services after normal office hours to people needing medical treatment.
Community Care is available Monday through Friday from 6 p.m. to 10
p.m. and is staffed by caring professionals. This team of professionals provides medical attention at substantially less than a regular Emergency Room Visit.

Illnesses and Injuries that meet Community Care Guidelines
|
* Upset stomach, such as nausea, vomiting and diarrhea
* Urinary Tract Infection
* Upper Respiratory Infection such as a cold, sore throat, or bronchitis
* Rash
* Eye Infections
* Toothache
* Sunburn
* Ear Infection
* Gout
* Insect Bite (uncomplicated)
* Tick Removal
* Simple Wound Care for abrasions and cuts limited to one area
* Simple sprain or strain (without x-rays)
* Follow-up exam after a visit to the ER
* Removal of stitches and staples

What is the cost and how do I pay?
|
The Cost of Community Care is $65 a visit, however, there may be other charges for medications.
All insurance is accepted, including Medicare and Medicaid and any co-payment or deductible due will be collected at the time of your visit.

Minor illnesses and injuries that DO NOT fit the guidelines of Community Care
|
* Multiple Minor Complaints with medical imaging testing or laboratory blood testing.
* Abrasions and/or cuts that are extensive or in multiple areas.
* Urinary Tract Infections or Upper Respiratory Infections with laboratory blood testing or x-rays.
* Sprain/Strain of Extremities that need x-rays
* Complicated Insect Bites with an infection
* Vascular or Migraine Headaches
* Upset stomach, such as nausea, vomiting or diarrhea that needs lab testing

Who Provides Community Care?

The Staff of Community Care is trained Medical Professionals dedicated to providing you compassionate and convenient care. The staff consists of a Nurse Practitioner, Medical Director and a Licensed Practical Nurse.

Where is Community Care Located?
|
Community Care is located in the hospital near the Cardiopulmonary Department. When arriving at the hospital, you can enter through the front doors, and signs will be posted to direct you to the appropriate area.

What if the Staff Determines I need Emergency Room Treatment?
|
|
After being seen in Community Care, if it is determined that you need more extensive care for your minor injuries or illnesses, the staff will transfer you to the Emergency Room and any co-payment or deductible collected will be applied to your Emergency Room visit.
